Dear Parents and Guardians,

We value the participation of your children in Katie Blessing Center’s Student Advisory Council. To promote our activities and achievements, we would like to occasionally feature council members in our social media posts.

Please indicate your preference below regarding your child’s appearance in our social media posts.

Program Description

I understand the Advisory Council involves students collaborating with Katie Blessing Foundation leadership on programs and initiatives.

Participation Details

My child may participate in meetings and activities outside school hours.

Voluntary Involvement

Participation is voluntary and can be discontinued by my child, myself, or Katie Blessing Center leadership at any time.

Media Consent

I consent to the use of my child's images for promotional purposes (no personal identifiable information will be used on social media).

Liability Release

I release The Katie Blessing Foundation and its staff from liability for my child's participation, except in cases of gross negligence or willful misconduct.

I understand and consent to my child's participation.
